30am Purpose of the position:
The purpose of this position is to provide professional or technical care of residents under the supervision of the registered nurse; to utilize the nursing process in planning and providing direct care to residents assigned; to observe and monitor care being provided by nursing staff during the shift on the assigned unit; to administer nursing care, medication, treatments and therapeutic measures as directed and according to Annandale Care Center policies and procedures; to document clearly and concisely according to accepted standards of nursing practice; to maintain and monitor that good housekeeping, safety, and infection control practices are met on the unit by all nursing staff; and to follow and monitor nursing staff in adherence to all facility policies and procedures, including safety, infection control, personnel, Vulnerable Adult Law and nursing department policies.
The LPN is responsible to understand and perform duties in accordance with the Nurse Practice Act.
Qualifications:
Current Minnesota licensure as an LPN or LVN or RN and CPR certification.
Graduated from an accredited school of nursing.
Previous experience in long term care preferred.
Able to read, write and speak enough English language to follow oral and written instructions.
Able to communicate effectively with staff, residents and families.
Able to meet the physical and emotional demands of the position as listed below.
Background check passed without restrictions.
Free from illegal use of drugs, and free from the use and effects of drugs and alcohol in the workplace.
Skills required:

Ability to understand and interpret resident charts, doctor's orders, care plans and medication orders.

Knowledgeable of nursing and medical practices and procedures; keeps current in medical management of conditions and best practices; and maintains knowledge of laws, regulations and guidelines.
Able to maintain confidential information of all aspects of the facility; follows HIPAA guidelines.
Ability to be flexible and deal with crisis/emergency situations in a calm, effective manner.
Able to use the telephone, computer, calculator, photocopier, fax machine and other office equipment.
Able to perform multi-task functions effectively and efficiently.
Must possess the ability to make decisions in accordance with Nurse Practice Act and to follow instructions and accept guidance.
Participates in coaching activities when staff performance warrants such action, notifies supervisor(s) of this action.
Serves as a resource for staff and others when necessary.

Nursing unit functions:
Understand and complies with applicable State and Federal regulations, the need for the regulation, and the importance of proper procedures and techniques.
Adheres to the laws of the Minnesota Board of Nursing Practice Act.
Listens to and gives complete shift to shift report and keeps lines of communication open with supervisors, co-workers and the on-coming shift.
Passes along report sheets to oncoming shift after completing required resident documentation.
Personally observes all assigned residents to monitor their physical, medical and psychosocial status.
Is continually aware of the condition of the unit regarding appointments, illness, calendar events, staffing patterns, etc.
Coordinates staff resources to appropriately meet the resident's changing needs.
Monitors that the nursing personnel on the unit are carrying out their assignments according to the plan of care and standards of practice; notifies the RN of any deviations occurring.
Assures appropriate staff are using the designated equipment, such as pagers, 2-way radios, transfer belts, mechanical lifts, etc.
Maintains a clean, safe and efficient work environment.
Monitors unit order for hazards, & safety issues.
Improvises temporary solutions to problems and hazards as needed.
Informs maintenance of problems or hazards needing to be fixed.
Attends resident care conferences upon request.
Shows respect for residents' rights and dignity, which includes respect for feelings, personal items, personal wishes, right to appropriate care, security, mental health, and in interacting with residents' families and friends.
Greets the resident as they wish to be addressed.
Informs residents of medications and procedures prior to onset.
Resident care functions / organizational skills:
Provides nursing care according to the care plan developed by an RN.
Organizes and sets priorities which include using time effectively to accomplish assigned duties within the shift parameters.
Maintains competence in all required skills within the facility.
Observes, records, and reports to a supervisor any changes in resident status.
Promptly contacts the physician and family to report resident changes in condition and/or contacts the RN on-call promptly for advice when needed; obtains and implements physician orders.
Documents these events.
Immediately reports any suspected abuse or neglect per facility policy and the Vulnerable Adult Law.
Administers prescribed medications and treatments per MD orders.
Orders medications when needed.
Successful in handling difficult situations, such as calming upset residents/family or caring for confused and/or combative residents in a dignified manner; able to handle difficulty between staff when needed.
Follows established shift routines regarding monitoring of vital signs, weights, I&O, administration of medications, treatments & documentation requirements.
Helps team members and works as a team player.
Complies with and understands emergency protocols and procedures and LPN responsibility during emergency situations.
Follows all DNR / DNI / Comfort Care / Full Code / CPR protocols.
Suggests and aides in implementing process or quality improvement ideas to supervisor; conducts and/or assists with training, education and/or audits when needed.
Charting and documentation functions:
Completes all required documentation and data collection in an accurate and timely manner.
Contributes to the development and updating of the care plan and staff assignment sheets by reporting pertinent observations of resident responses and needs; and suggests any recommendations to the RN.
Accurately transcribes physician's orders per facility protocol and forwards necessary information to other departments and pharmacy.
Documents and reports all incidents, accidents, and near misses that occur, involving residents, staff and visitors.
Personal and professional growth:
Maintains current CPR and Board of Nursing certification during employment.

Ability to endure prolonged walking, sitting, standing, pulling, bending, stooping, balancing and lifting (25-35 pounds) movements.
Occasional climbing, crawling, crouching and kneeling movements.
Ability to adapt to changes in daily workload and work hour schedule.
Able to perform repetitive motions of wrists, hands, finger, pushing, pulling, grasping, reaching, and walking.
Able to adequately hear residents, staff, alarms, devices, etc.
Able to appropriately identify residents, call lights, unusual occurrences on unit, at a distance of 125 feet.
Able to correctly differentiate between blood and other body fluids.
Other sensory and communication activities essential include the ability to feel, smell and speak appropriately.
Able to read small print in typed or written material.
Able to perform mathematical functions, including fractions, decimals and percentage calculations.
Possess sound reasoning skills; able to assist in solving practical and intellectual problems.
Concentrates with frequent interruptions, remembers multiple messages, information, and tasks during the regularly scheduled shift.
Must be in good general health and demonstrate emotional stability.
Environmental job demands / working conditions:
The LPN / LVN is at significant risk for potential exposure to blood borne pathogens such as Hepatitis B, Human Immunodeficiency Virus (HIV), etc.
Potential for exposure to hazardous chemicals as identified by MSDS manual.
Potential for exposure to bodily injuries due to unpredictable behaviors of residents.
Potential for exposure to infectious bodily fluids of residents, staff or others.
Potential for respiratory and chemical hazards.
Occasional exposure to microbial and other infectious agents in care of ill/dying residents.
Occasional exposure to latex, plastic, vinyl, and/or materials which are used for personal protective equipment.
Reports occupational exposures to needle sticks, blood and body fluids, infectious materials, and hazardous chemicals in accordance with the facility protocol.
AnnandaleCareCenter provides nursing care 24 hours a day, 7 days a week.
Shift lengths vary, most positions involve working every other weekend and holiday.
Uniforms are any colored scrub shirts and pants.
Shoes should have good support, non-skid soles, and closed toes.
Nametags must be worn at all times.
Work is performed in a well-lit, heated/air conditioned nursing unit.
